Modified Spring "Higher-Risk" Boys Lacrosse
April 19, 2021
RE: COVID Testing for High-Risk Sports
Dear Middle School Boys Lacrosse Families,
I hope you all had a great weekend. As we prepare for the spring season, below you will find important information regarding our COVID-19 testing which is required for all of our higher-risk sports. We will also review these protocols in detail at tonight’s parent meeting.
COVID-19 testing for Boys Lacrosse participants and coaches will take place on Wednesday, April 21st from 1:00pm - 2:30pm. Parents are required to bring their child for this first testing session.
Students must be COVID-19 tested on April 21, 2021 in order to be eligible for participation.
Please sign your child up for a testing appointment by 12:00 pm on Tuesday, April 20th, using this COVID TEST APPOINTMENT FORM. Appointments slots are only available for athletes and coaches of our higher-risk sports. You must also complete and submit all three forms found below.
Thank you for your cooperation and please reach out to me with any questions.
Sincerely,
Mr. Jason Semo
Director of Athletics, Health and Physical Education

Below you will find a map of the two stations that we will use to support our COVID19 testing process.
Please understand that no student-athlete will be permitted to participate in a high-risk sport unless they are (1) tested through the process described above, or (2) if they produce COVID-19 testing results from a lab on or after our weekly testing date (students waiting for test results from an outside provider will not be permitted to participate).
We thank you for your patience, support, and understanding.
