Chappaqua Central School District's COVID Testing Management System (CTMS)

Welcome to the CCSD COVID Testing Management System!

We are pleased to offer COVID-19 testing onsite for our staff and students.   The District has a “Limited Lab Site License” which enables our nursing staff to administer the BinaxNOW Ag Card

The District will begin required weekly testing of unvaccinated staff starting on Monday, 9/27/21, from 3-5pm at Seven Bridges Middle School.  The testing of unvaccinated athletes will also continue weekly at the same time, also at Seven Bridges.  Vaccinated staff may also choose to sign up for weekly COVID-19 testing conducted by CCSD.

Testing for K-12 unvaccinated students (who are not CCSD athletes) will be optional, and offered, so long as the District is identified by the CDC as having moderate, substantial, or high transmission rates (Commissioner’s Determination on COVID-19 Testing, pg.2).

***The appointment process will close at 8am on the date we are testing, for individuals who are not required to receive a weekly COVID-19 test.***


PRIOR TO YOUR ARRIVAL AT SEVEN BRIDGES FOR COVID-19 TESTING

  • SCHEDULE COVID-19: Test Appointment using the link above (only individuals registered in advance will be tested by CCSD).
  • COMPLETE OPEROO: In the morning (vaccinated staff who are electing to test will be screened onsite).

WHEN YOU ARRIVE AT SEVEN BRIDGES FOR TESTING

  • Please stay in your vehicle.
  • Please print and bring the Consent for Testing Form (from above).
  • You must be asymptomatic to be tested.

    Please  DO NOT come on campus if you are exhibiting any COVID-19 symptoms. Symptoms include, but are not limited to: fever, headache, congestion or runny nose, cough, loss of taste or smell, nausea or vomiting.
  • Please be on time for your testing appointment.
  • Please wear a mask at all times.

AFTER TESTING AT SEVEN BRIDGES MIDDLE SCHOOL

  • Promptly leave the campus.
  • We will contact you immediately by phone if your child’s test result is positive. In the event your child is positive, they will be excluded from school, the District will notify the WCDOH, and we will begin contact tracing.
  • If you would like a paper copy of your child’s negative test result for your records, please contact your school's nurse.

Private Testing Reporting System   

To be exempt from weekly testing onsite, you may present a negative COVID-19 rapid or PCR test provided the following conditions are met: 

  • The test is administered Saturday, Sunday, or Monday. 
  • The test is administered by a medical professional or supervised by a medical professional and is either an FDA or NYSDOH approved test.
  • The results are certified by a lab or medical facility that is NYS Certified.
  • The results are submitted to CCSD before our onsite testing window closes (ex. results must be received by 5pm Monday, if we are testing Monday, or by 5pm Tuesday if we are testing Tuesday).
