Regulation 3035: Records Management and Access

Requests for records shall be made in writing to the Records Access Officer, P.O. Box 21, Chappaqua, New York 10514 or by email to Such requests shall contain the name of the person making the request, his/her address or email address and a reasonable description of the records being requested.
